研究案内

獨協医科大学上部消化管外科では上部消化管疾患のみならず、消化器・一般外科および小児外科疾患に関する診療を行っております。患者さん個々の状態に応じて綿密な治療計画のもと、最良の治療を行うことを最大の目標としております。癌の患者さんに対しては、最善の治療結果を得るために手術に加えて化学療法（抗癌剤治療）なども施行しています。また放射線治療の対象となる患者さんも当科で診療をさせて頂いております。これらの診療の過程で得られる情報をもとに様々な研究を行い、医学の発展に貢献すべく日々努力しております。

1. 手術に関する研究

最近の外科手術の進歩は目覚ましく、以前はおなかを大きく開いて手術をしていた疾患の多くに内視鏡手術（腹腔鏡手術）が導入されています。当科では胃癌や大腸癌に対しては標準的に腹腔鏡手術を行っており、その進化形としてロボット支援手術も多数行っております。また胸部操作が中心となる食道癌に対してもロボット支援手術を含めた胸腔鏡手術を施行しています。また一方で、通常では手術の対象とならないような進行癌に対しても他の診療科と連携して拡大手術を行っています。こういった手術の結果を検証し、将来的にさらに手術が進歩するように努力を重ねております。

2．診断に関する研究

手術や化学療法などを行う前には血液検査や消化管内視鏡（胃カメラ、大腸カメラなど）検査、超音波検査、CT、MRIあるいはPET検査など、必要に応じて様々な検査を行います。これらの検査から得られる情報で、その後の治療法を決定したり、時にはその後の治療効果などを予測することも可能になることがあり、またそれらの結果をもとにさらなる検査法を生み出すことも可能となります。患者さんに行った検査の結果を無駄にせず、将来の発展につながるように研究を行って参ります。

3．補助療法（化学療法、放射線療法など）に関する研究

当科では手術を中心とした治療を行っておりますが、手術との組み合わせで化学療法や放射線療法も行っております。これらの治療を手術治療の対象とならない患者さんに対して行うこともあり、また再発した場合などでも同様に行う場合があります。こういった治療法の成果を広く公表していくことで、どういった患者さんにどのような治療を行うのが適切であるかなど、広く情報共有を行うことができます。またそういった成果をもとに、新しい治療法を提案することも可能となります。新規に開発された薬剤や、新しい組み合わせの治療法なども積極的に導入し、患者さんにとって少しでもいい生活を送ることができるよう、全力でサポートをしてきたいと考えています。

4．基礎的研究

癌をはじめとする疾患では、患者さんの組織や血液などを用いて基礎的な実験を行うことによって将来的な治療の進歩に結び付けるこということも大切な作業のひとつです。個別の遺伝子やその産物に注目した研究や、複数の遺伝子を網羅的に解析する研究など、その手法は様々ですが、我々は従来から手術で摘出した組織などを用いて基礎的な研究を行って参りました。今後も患者さんから頂いた貴重な組織を無駄にすることなく、将来の医学の発展のために研究を行っていきたいと考えております。

5．症例報告

普段あまり目にすることのない稀な疾患は、診断や治療に関する情報が少なくいため、論文などで多くの医療関係者に知ってもらうことが非常に重要です。そうすることで不幸にして稀な疾患にかかってしまった患者さんに適切な治療法を提供することが可能となります。稀な疾患に対する治療を行った際には報告を行っております。
